 Quini-ne is one of the oldest antimalarial drugs. As early as the 15th century, Cinchona bark containing quini-ne was widely used to treat malaria. In addition to medicinal use, it can be used as the detection agent of metal ions such as bismuth and platinum in analytical chemistry, and can also be used as the extraction agent of racemic organic acids. The oral absorption of this product is fast and complete, and the blood concentration can reach the peak in 1 ~ 3 hours. The binding rate to plasma protein is about 70%. Sulfate for oral use and hydrochloride for injection. Until the 1920s, it was a better treatment for malaria. But if used improperly, it can also be toxic, causing headaches, tinnitus, diarrhea, skin rash, vision and hearing disorders. It has only inhibitory effect on promalaria, but no killing effect, and patients are still prone to the disease after recovery. For this reason, scientists are still looking for more effective antimalarial drugs, such as alacrine, parmalinoline and chloroquin-e. Changshanine, which can be extracted from Changshanine, a plant from China, is 100 times more effective against malaria than quini-ne. However, it cannot be taken directly because of its toxicity. Adverse reaction:
1, Cinchona reaction: daily dose of quini-ne more than 1g or a little longer can appear, manifested as nausea, vomiting, tinnitus, headache, vision and hearing loss, generally can be recovered after withdrawal.
2. Atypical reactions: acute hemolysis, dermatitis, pruritus, angioneurotic edema, bronchial asthma, etc. In a few patients with falciparum malaria, a small dose of Chemicalbook quini-ne can be seen chills, high fever, vomiting, hemoglobinuria, uria and other acute hemolysis, known as black urine fever, severe cases can be fatal.
3, intravenous injection, can inhibit the heart caused by blood pressure drop, even shock life-threatening, so it is strictly prohibited. Static drops should also be used with caution. Muscle injection is easy to cause tissue necrosis, in addition to oral, generally do not use.
First aid:
1. Patients with abnormal ECG or Cinchona syndrome should stop taking medicine immediately and receive symptomatic supportive treatment.
2. Stellate nerve blocking therapy can be used in the early stage of amaurosis except drug withdrawal.
3, in case of intravascular hemolysis and black urine fever, the drug should be stopped immediately. Absolute bed rest, intravenous injection of glucose solution and oligosaccharides to improve microcirculation; Chemicalbook Sodium 5% bicarbonate 250 ~ 500ml intravenous drip to prevent renal tubule blockage; Adrenocortical hormone is used to control hemolysis; Patients with severe anemia may receive blood transfusion as appropriate. Patients with acute renal failure should be treated accordingly.
4. After oral administration of quini-ne, it can be effectively purified by hemoperfusion treatment before it is completely absorbed and distributed to tissues. Hemodialysis or peritoneal dialysis may be used in renal failure.
